GUIDE Program Manager
Position Overview
As the GUIDE Program Manager at Ceresti Health, you will be responsible for the operational excellence and continuous improvement of our CMS GUIDE Model implementation. This is a hands-on operational role focused on managing day-to-day program execution, ensuring compliance with all GUIDE requirements, and achieving critical performance metrics. You will lead a team of program coordinators, collaborate closely with enrollment and engagement teams, and contribute to the ongoing development of our technology platform to support program delivery.
This role requires someone who excels at managing complex operational processes, meeting strict regulatory timelines, and driving cross-functional collaboration to deliver exceptional outcomes for families affected by dementia.

Key Responsibilities
Program Operations & Execution (30%)
- Manage day-to-day operations of the GUIDE Model program, ensuring seamless delivery of all required services
- Monitor and optimize existing processes to improve efficiency while maintaining compliance
- Oversee the delivery of all GUIDE-mandated elements including:
- Comprehensive assessments within required timeframes
- Care plan development and quarterly updates
- 24/7 caregiver support access
- Home visit and respite services coordination and documentation
- Caregiver training and education programs
- Implement continuous process improvements based on operational data and team feedback
- Manage program workflows across multiple stakeholders to ensure smooth execution
Team Leadership & Coordination (20%)
- Lead and manage a team of program coordinators responsible for GUIDE implementation
- Conduct daily/weekly huddles to review priorities, address challenges, and ensure alignment
- Develop training materials and standard operating procedures for team members
- Manage workload distribution and capacity planning across the coordinator team
- Conduct performance reviews and provide ongoing coaching and development
- Foster a culture of operational excellence and continuous improvement
Cross-Functional Collaboration (15%)
- Partner closely with enrollment team to ensure smooth beneficiary onboarding
- Collaborate with engagement team to maintain required touch points and caregiver support levels
- Work with coaching team to ensure delivery of education and support services
- Coordinate with clinical team on assessment completion and care plan development
- Interface with finance team on respite services tracking and billing
- Align with compliance team to ensure all regulatory requirements are met
Technology & System Enhancement (10%)
- Contribute to ongoing software development by providing operational insights and requirements
- Partner with IT team to optimize platform functionality for GUIDE workflows
- Identify automation opportunities to improve efficiency and reduce manual processes
- Test and validate new features before deployment
- Gather user feedback from coordinators and stakeholders to inform system improvements
- Ensure technology solutions support compliance and reporting requirements
Required Qualifications
Education & Experience
- Bachelor's degree in Healthcare Administration, Public Health, Business Administration, or related field
- 4-6 years of experience in healthcare program management, with emphasis on operational execution
- Direct experience with CMS programs, Medicare Advantage, or value-based care models required
- Proven track record of managing complex healthcare operations and achieving performance metrics
- Experience leading and developing operational teams
