Position Overview

Ceresti Health seeks an experienced Director of Engagement to lead our comprehensive caregiver and patient engagement strategy, brain health initiatives.  This transformational role will combine clinical expertise, operational excellence, and data-driven decision-making to maximize caregiver participation and retention across all touchpoints of our service delivery. Under your leadership we will be able to deliver whole-person, whole-family care at scale while expanding our impact to more families living with dementia.

This is a transformational role: You will spearhead two critical initiatives:

  1. Add comprehensive care coordination to our proven coaching program - Expand our current coaching model into a comprehensive team-based model.
  2. Maximize engagement - Build a multi-touchpoint system that ensures sustained caregiver participation from early cognitive changes through advanced stages of dementia

Your leadership will ensure that every family receives continuous support through brain health education, early-stage programs, coaching, and eventually full care coordination services. You'll oversee the strategic roadmap for this evolution while maintaining excellence in current program delivery.

Key Responsibilities

Care Coordination & Service Evolution

  • Transform proactive coaching capabilities into an integrated care management model with expanded partnerships and interdisciplinary team structures

Strategic Program Leadership

  • Drive comprehensive engagement strategies to maximize participation and retention
  • Create multi-modal pathways and establish performance metrics across all initiatives

Team Development & Culture

  • Lead diverse team of specialists, coaches, and coordinators
  • Build training programs, career pathways, and retention strategies
  • Foster culture of empathy and innovation while preventing burnout

Data & Performance Optimization

  • Leverage analytics to identify trends, optimize strategies, and predict at-risk populations
  • Implement testing frameworks and regular KPI reporting
  • Track program outcomes and team satisfaction metrics

Innovation & Technology

  • Partner on digital tool enhancement and platform development
  • Explore emerging technologies and behavioral science applications to enhance engagement

Required Qualifications

Education & Experience

  • Bachelor’s degree in a related field
  • Minimum 7+ years of experience in healthcare engagement, care coordination, behavior change, or program management
  • 3+ years of leadership experience managing teams and complex programs
  • Demonstrated experience in dementia care, brain health, or caregiver support programs
  • Experience developing or scaling care coordination programs preferred

  • Program Development: Ability to transform and scale programs from pilot to full implementation
  • Data-Driven Decision Making: Strong analytical skills with ability to translate data into actionable engagement strategies
  • Innovation Mindset: Creative problem-solving with ability to design and test new engagement approaches
  • Empathetic Leadership: Genuine passion for supporting caregivers with ability to inspire teams
  • Systems Thinking: Ability to design scalable engagement and care coordination systems and processes
  • Communication Excellence: Outstanding verbal and written communication skills across diverse audiences

Preferred Qualifications

  • Master’s degree in a related field
  • Experience in value-based care models or CMS innovation programs (GUIDE Model experience highly valued)
  • Care coordination certification (CCM, CMAC, or similar)
  • Experience with Medicare Advantage, managed care, or ACO environments
  • Background in adult learning principles and curriculum development
  • Experience with community outreach and partnership development
  • Knowledge of healthcare reimbursement models and billing for care coordination services
