We're at the forefront of transforming dementia care nationwide — and the Account Manager for Medicare Advantage Plans is one of the most important roles in making that real.

Position Overview

The Medicare Advantage (MA) Plans Account Manager is a high-ownership, relationship-intensive role at the intersection of performance management, provider and plan engagement, and organizational change. You will serve as the primary partner for Medicare Advantage plans partnering with Ceresti to strategically leverage our caregiver-centric dementia care model to impact one of the costliest, most underserved cohorts in their member populations.

This role is responsible not just for account health, but for building the kind of trust that allows Ceresti to proactively enroll eligible members on behalf of those plans. MA plans operate within a highly regulated, quality-driven environment — with medical directors, population health teams, care management staff, and plan leadership who each carry distinct priorities. Your job is to earn and maintain the trust of all of them, and to translate that trust into enrollment performance, program adoption, and measurable outcomes.

This is not a reactive account management role. You'll succeed here if you're energized by complexity, comfortable navigating the operational realities of managed care, and deeply motivated by what it means to get this right for families living with dementia.

Key Responsibilities

MA Plan Partner Relationship Management

  • Serve as the primary account owner for assigned Medicare Advantage Plan partners from post-implementation through steady state.
  • Build strong, trusted relationships with plan leadership, population health and care management teams, medical directors, and quality stakeholders.
  • Develop a deep understanding of each plan's member population, benefit design, network structure, and strategic priorities — and connect Ceresti's model to what matters most to them.
  • Act as an internal liaison at Ceresti for the MA Plan, ensuring the partner’s needs, constraints, and priorities are clearly understood, communicated, and appropriately addressed.
  • Support contract adherence, value-based arrangement alignment, and ongoing partnership health.

Program Performance & Growth

  • Own enrollment performance for assigned MA Plan partners, including monitoring referral flow from care management teams, outreach effectiveness, and caregiver activation.
  • Identify opportunities to improve member uptake through care management team and customer service team education, workflow refinement, and targeted engagement strategies.
  • Partner with internal teams to troubleshoot enrollment barriers and optimize operational processes.
  • Support discussions around performance results, cost and quality outcomes, and opportunities for program expansion or contract growth.

Product & Care Management Team Enablement

  • Drive awareness and buy-in across assigned plan networks – including direct engagement with care managers, utilization management teams, and network providers to establish Ceresti’s model as a trusted component to existing are coordination efforts.
  • Coordinate member-facing and provider-facing marketing materials, training, and communications in collaboration with Marketing & Engagement teams.
  • Ensure Plan partners understand Ceresti’s caregiver-centric approach and how it complements—not replaces—existing care management programs.

Enrollment Stewardship

  • Build and maintain the plan-level trust and governance alignment required for Ceresti to conduct proactive outreach to eligible members on behalf of partner plans. Own the internal and external approvals, workflows, and communication protocols that make proactive enrollment operationally viable, compliant, and clinically credible.
  • Assist in the creation and maintenance of outreach materials such as postcards, flyers, a co-branded informational website and enrollment scripts.

Operational Coordination

  • Coordinate closely with Implementation, Coaching, Enrollment and Operations teams to ensure smooth ongoing operations.
  • Support data-sharing workflows, membership file refreshes, and outreach approvals in alignment with MA Plan governance and CMS compliance requirements.
  • Monitor operational SLAs, outreach cadence, and member experience to ensure a high-quality, compliant program.
  • Assist with generating monthly invoices based on reporting and measured member engagement each month.

Reporting & Communication

  • Lead recurring partner check-ins and business reviews with assigned MA Plan partners.
  • Deliver clear, data-driven updates on enrollment, engagement, outcomes, and operational performance – including quality metrics and cost impact relevant to MA Plan priorities.
  • Translate complex data into concise, executive-ready insights for plan stakeholders, including medical directors and VP-level audiences.

Required Qualifications

Education & Experience

  • Bachelor’s degree in a related field.
  • 5+ years of experience in account management, client success, provider relations, or partnership management within healthcare.
  • Experience working with Medicare Advantage Plans, managed care organizations, or value-based care entities.
  • Strong understanding of MA Plan operations, care management workflows, and member population dynamics.
  • Proven ability to manage complex, multi-stakeholder relationships across clinical, operational, and executive audiences..
  • Exceptional communication, organization, and follow-through skills.
  • Exceptional problem-solving skills.

Preferred Qualifications

  • Experience supporting supplemental benefit program, chronic condition management models, or CMS innovation programs within MA Plans.
  • Familiarity with caregiver support, dementia care, home care, or care management models.
  • Experience presenting performance data and outcomes to executive and clinical audiences.
  • Comfort working in a high-growth, fast-moving startup environment.
  • Experience with HubSpot is a plus.
