Ceresti Health and PacificSource to Activate Family Caregivers, Improve Outcomes for Medicare Members living with Dementia
Ceresti Health announced today it has deployed its proven dementia care program to family caregivers of PacificSource Medicare Advantage and DSNP (Dual Eligible Special Need Plan) members living with dementia. By increasing the knowledge, skills and confidence of family caregivers, Ceresti’s program aims to reduce avoidable and often-harmful member hospitalizations.
