Ceresti Health and PacificSource to Activate Family Caregivers, Improve Outcomes for Medicare Members living with Dementia
Ceresti Health announced today it has deployed its proven dementia care program to family caregivers of PacificSource Medicare Advantage and DSNP (Dual Eligible Special Need Plan) members living with dementia. By increasing the knowledge, skills and confidence of family caregivers, Ceresti’s program aims to reduce avoidable and often-harmful member hospitalizations.

PacificSource and Ceresti Health Demonstrate Significant Cost Savings in Medicare Advantage Members Living with Dementia
New claims-based results from PacificSource and Ceresti Health show that Ceresti's dementia management program, delivered through Ceresti's participation in the CMS GUIDE Model, achieved a 42% reduction in medical costs and statistically significant savings of $523 per member per month for Medicare Advantage members living with dementia. The findings demonstrate that activating family caregivers as an extension of the care team can meaningfully reduce avoidable hospitalizations and ER visits in a Medicare Advantage environment under growing cost pressure.

Griswold and Ceresti Health Partner to Expand Dementia Caregiver Support Nationwide
Griswold is partnering with Ceresti Health to expand dementia caregiver support for families across the country. Through the partnership, Ceresti's GUIDE-aligned dementia care program will connect eligible families with Griswold's nationwide network of caregivers for in-home respite and support services. Together, the organizations aim to reduce caregiver strain, improve care coordination, and help people living with dementia remain safer at home. Ceresti Health is a nationwide GUIDE Participant.
