COMPLEX PATIENTS UNABLE TO SELF-MANAGE ARE THE MOST EXPENSIVE

Patients unable to self-manage their chronic conditions include those with Alzheimer’s Disease and other dementias, stroke, or a traumatic brain injury. These patients have difficulty (i) following instructions, (ii) implementing physician care plans and (iii) articulating their symptoms, including pain.

Traditional patient-centric disease management programs are not effective for patients unable to self-manage their chronic conditions due to having dementia.

Dedicated Caregiver Coach

Family caregivers providing care for your members are matched to a dedicated Ceresti caregiver coach based on their social style, program goals, and member chronic conditions.

Ceresti Tablet and Mobile Apps

Caregivers access a personalized curriculum on the (included) Ceresti tablet and communicate with their coach. Family members use their mobile app to send pictures and messages to the tablet.

Care Management Portal

Caregiver coaches use our cloud-based HIPAA compliant care management portal to track progress from multiple caregiver/patient dyads and communicate with caregivers.

PRICED FOR YOUR SUCCESS

We offer a fixed monthly fee that allows you to enroll all eligible members into one of our programs.

We offer performance-based pricing where some (or all) of our fees are at risk based on our ability to achieve engagement and outcomes, such as reducing the utilization of acute or post-acute healthcare services and/or associated costs, or reducing the number of preventable care transitions.

We offer traditional transaction-based pricing where you pay a fixed fee for each member who enrolls in one of our programs.
